Enrollment Forms

Our enrollment forms are now online!  The 2010-2011 Enrollment Form and Emergency Information Card are both available as editable pdf files.  We hope that you will find these easier to complete, especially if you have more than one child enrolled at RMA. 

Please note:  there are two copies of the Emergency Card in the document.  Please fill out one front (contains the RMA logo) and one back for each child. 

The pdf files can be typed into and then printed.  Please either mail or drop off the completed form to the school.  You also have the option to print the blank form and fill it in by hand and mail it back or drop it off.

To print, fill in the requested fields and select the print button at the bottom of the last page or go to your browser window, choose File and then Print.  If you would like to print it to fill in by hand, please use the same method to print a blank form.

Please complete the forms as soon as possible and return them to us.  We MUST have all completed forms by the time students return to school on August 2.
